Abstract (German)
Die infektiöse Endokarditis ist eine Erkrankung mit hoher Letalität, etwa 20 % versterben nach Diagnose innerhalb eines Jahres. Bei 20 bis 55 % kommt es zu neurologischen Komplikationen. Der Einfluss dieser auf das Outcome der Patienten zeigt sich in der Literatur mit widersprüchlichem Ausmaß, sodass Unsicherheiten bei der Therapie bestehen (z. B. bezüglich des optimalen Operationszeitpunktes). Bei intrakraniellen Blutungen empfehlen die ESC-Leitlinien von 2015 eine generelle Verschiebung der Operation um 4 Wochen, jedoch auf widersprüchlicher Evidenz basierend und ohne weitere Differenzierung der Blutungen. Mit diesem Hintergrund sollten bei IE-Patienten mit verschiedenen neurologischen Komplikationen (bei Beachtung der Subtypen intrazerebraler Blutungen) Mortalität und neurologisches Outcome verglichen werden.
Hierfür wurden bei einer konsekutiven Serie von 120 Patienten, die in den Jahren 2013 bis 2016 mit IE am UKR behandelt wurden, verschiedene kardiologische, mikrobiologische, laborchemische, neurologische und neuroradiologische Parameter erhoben. Der neurologische Zustand wurde mit der modifizierten Rankin-Skala (mRS) quantifiziert. Endpunkt der Studie, welcher nach durchschnittlich 657 Tagen auftrat, war der Tod oder Zeitpunkt des telefonischen Follow-ups. Die Patienten wurden in zwei Gruppen unterteilt, operative (OT, n = 88) und konservative (MT, n = 32) Therapie. Die operative Therapie fand durchschnittlich 11 ± 17 Tage nach Aufnahme statt.
Die am meisten betroffenen Klappen waren die Aorten- (bei 58 %) und die Mitralklappe (bei 49 %). Erreger konnten in 85 % der Fälle mittels Blutkultur nachgewiesen werden, darunter Staphylokokken (44 %), Streptokokken (30 %) und Enterokokken (15 %). Echokardiographisch zeigte sich in 78 % der Fälle eine endokarditische Läsion. Neurologische Komplikationen zeigten sich in insgesamt 47 Fällen, 35 (40 %) vs. 12 (38 %) in der OT- vs. MT-Gruppe. Zerebrovaskuläre Komplikationen gab es bei 33 % (OT) vs. 34 % (MT). Intrazerebrale Blutungen hatten 11 (13 %) der OT- und 7 (22 %) der MT-Gruppe. Die Gesamtmortalität bei einem Follow-up nach durchschnittlich 1,8 ± 1,3 Jahren lag bei 25 % (17 % vs. 47 %). Ein-Jahres- und Gesamtmortalität waren in der MT-Gruppe signifikant höher. Bei Vorhandensein einer CVC lag die Mortalität bei 21 % vs. 45 %. Das Auftreten von Apoplex im Verlauf war signifikant häufiger in der MT-Gruppe (25 % vs. 8 %). Es zeigte sich insgesamt eine neurologische Verbesserung von durchschnittlich 0,2 Punkten in der mRS bei der OT-Gruppe und eine Verschlechterung von durchschnittlich 1,0 Punkten in der mRS bei der MT-Gruppe. Bei operierten Patienten war die Gesamtmortalität der Patienten mit asymptomatischen Läsionen (mRS = 0) bei 0 %, mit leichter bis mittelschwerer Symptomatik (mRS = 1-3) bei 19 % und mit schwerer Symptomatik (mRS = 4-5) bei 33 %. Lagen intrazerebrale Blutungen vor, lag die Mortalität bei 19 %. Bei operierten Patienten ohne CVC lag sie bei 15 %.
Summa summarum zeigte sich trotz zügiger Operation bei Patienten mit neurologischen Komplikationen (schwere neurologische Symptomatik bei Baseline ausgenommen) kein schlechteres Langzeitoutcome und ein günstiger neurologischer Verlauf. Mit Beachtung von Limitationen der Aussagekraft durch geringe Patientenzahlen einzelner Subgruppen konnte auch bei größeren intrazerebralen Blutungen kein Hinweis auf Risikosteigerung bezüglich Mortalität oder neurologischer Verschlechterung durch frühe chirurgische Intervention gefunden werden. Die Daten zeigen vielmehr, dass eine operative Therapie das Outcome, insbesondere auch bei Patienten mit intrazerebraler Hämorrhagie eher günstig beeinflusst. Die Empfehlung der ESC-Leitlinie, bei intrakraniellen Blutungen die Operation um 4 Wochen zu verschieben, erscheint nicht generell sinnvoll, sondern sollte für jeden individuellen Fall durch das „Endokarditis-Team“ bewertet und mit größeren Studien, welche zudem zwischen verschiedenen Blutungstypen/-graden unterscheiden, überprüft werden.

The infectious Endocarditis is a disease with high lethality. 20 percent of the affected die within a year after the diagnosis. Neurological complications are common and occur at 20 to 55 percent. Those complications affect the outcome of the patients which is shown in contradicting statements in the literature. Therefore, uncertainty arise concerning the treatment (e.g. the optimal time of operation). In case of intracranial hemorrhage, the ESC-guidelines of 2015 recommend that the operation should be postponed by 4 weeks even though this recommendation is based on contradictory evidence and further differentiation of the bleeding is missing. Due to this background knowledge, the mortality and neurological outcome of IE-patients with various neurological complications should be compared (and the subtypes of intracerebral hemorrhage) should be considered.
Therefor various cardiac, microbiological, neurological and laboratory parameters were collected in a consecutive series of 120 patients, who were treated regarding their infectious endocarditis at the University Hospital of Regensburg in the years 2013 to 2016. Their neurological condition was quantified with the modified Rankin-Scale (mRS). The studies ended after an average of 657 days either with death or with follow-ups by phone. The patients were divided into two groups, the operative (OT, n = 88) and conservative (MT, n = 32) therapy. The operative therapy started after an average of 11 +/- 17 days once the patient was admitted to the hospital.
The most affected valves were the aortic (58 %) and the mitral valves (49 %). Pathogens could be detected in 85 % of the cases within the blood culture, including staphylococci (44 %), streptococci (30 %) and enterococci (15 %). In 78 percent endocardial lesions were shown using echocardiography. Neurological complications appeared in 47 cases, 35 (40 %) in the OT- versus 2 (38 %) in the MT-Group. Cerebrovascular complications appeared at 33 % (OT) versus 34 % (MT). 11 (13 %) of the OT- and 7 (22 %) of the MT-group had intracerebral complications. The overall mortality at a follow-up, on average after 1,8 +/- 1,3 years, was at 25 % (17 % vs. 47 %). One-Year-Mortality and overall mortality were significantly higher in the MT-group. If CVC was present the mortality was 21 % vs. 45 %. The appearance of Apoplexy in the progress was significantly higher int the MT-group (25 % vs. 8 %). Summed up there was a neurological improvement of 0,2 points on average in the mRS of the OT-Group and a deterioration of 1,0 points in the mRS of the MT-group. For operated patients with asymptomatic lesions (mRS = 0) the overall mortality was at 0 %, for patients with mild to moderate symptoms (mRS = 1-3) at 19 % and for patients with severe symptoms (mRS = 4-5) at 33 %. In case of intracerebral hemorrhage, the mortality was at 19 %. For operated patients without CVC at 15 %.
Summarizing it appeared that, despite speedy operations for patients with neurological complications (severe neurological symptoms except Baseline), the long-term outcome wasn’t worse and the neurological course was better. Considering the limitation of the expressiveness, due to the low count of patients of single subgroups, no indication could be found of increased risk for mortality or neurological deterioration by early surgical intervention even at major intracerebral bleeding. Rather the data is showing that an operative therapy affects the outcome, especially for patients with intracerebral hemorrhage, positive. The recommendation of the ESC-Guideline to shift the operation by 4 weeks in case of intracranial bleeding doesn’t always seem to be the best way. Each individual case should be rated by the “Endocarditis-Team” and verified with bigger studies which additionally differs between the various types and degrees of bleeding.
