Die rhegmatogene Ablatio retinae ist eine komplexe Erkrankung auf dem Gebiet der Augenheilkunde und bedarf in der Regel einer operativen Therapie. Hierzu haben sich mehrere Operationsmethoden etabliert. Allerdings ist sich die internationale Literatur aber noch nicht einig, welches Verfahren bei welchen Voraussetzungen am besten geeignet ist.
Diese Arbeit hat retrospektiv die Op-Ergebnisse und verschiedene Einflussfaktoren auf den anatomischen und funktionellen Erfolg einer großen Kohorte von 619 Fällen einer rhegmatogenen Netzhautablösung mit gleichzeitiger Abhebung der Makula untersucht, die zwischen Januar 2007 und Juni 2012 an der Universitätsaugenklinik Regensburg operativ versorgt wurden.

58% der Patienten waren Männer, das Alter der Patienten betrug im Durchschnitt 63,5 Jahre. Der mittlere präoperative Visus betrug 1,5 log(MAR). Bei 60% der Patienten war der Linsenstatus zum Zeitpunkt der Netzhautablösung phake. Die primäre Wiederanlagerate nach einer Operation betrug 87%.

Es konnte gezeigt werden, dass die große Varianz der Ausgangsvoraussetzungen im Hinblick auf den Visus präoperativ, den Linsenstatus, der Größe der Ablösung, des Alters der Patienten oder der Art und Anzahl der Netzhautläsionen Einflüsse auf die Wahl der operative Versorgung hat.

Folgende Faktoren waren signifikant mit einem anatomisch erfolgreichen Ergebnis nach einer Operation assoziiert:
- präoperativer Visus
- geringe Zahl von Netzhautlöchern
- Erfahrung des Operateurs

Von 408 Augen konnten Daten zum Visus ein Jahr postoperativ erhoben werden. Dieser betrug im Durchschnitt 0,47 log(MAR).
Folgende Faktoren waren signifikant mit einem guten Visus ein Jahr postoperativ assoziiert:
- Geringer Schweregrad der Ablatio
- Junges Alter des Patienten
- Vorliegen eines Hufeisenrisses
- Präoperativ guter Visus
- Vorliegen nur eines Netzhautloches
- Plombe oder ppV mit Gas als Op-Methode

Die Daten der vorliegenden Arbeit zeigen, dass die Behandlung der rhegmatogenen Netzhautablösung mit modernen Techniken und Konzepten gute Ergebnisse bringt, aber noch nicht alle Probleme wirklich gelöst sind.
Aktuelle Trends der Ablatiochirurgie in der Literatur werden bestätigt.

The rhegmatogenous Ablatio retinae is a severe ophthalmological Illness and needs normally an operation for therapy. Several operation methods are established, but the international literature is not sure about which treatment is the best in specific cases.
This work analyzed retrospectively the results and influencing factors on the anatomical und functional success of 619 cases of rhegmatogenous retinal detachment with detached macula. All cases were operated between January 2007 and June 2012 at the university eye hospital Regensburg.
58% of the patients were men, averaged age was 63,5 years. Averaged preoperative visus was 1,5 log(MAR). Lens state at time of operation was phake in 60% of the cases. The primary attachment rate after the first operation was 87%.
It could be displayed, that the huge range of the preoperative conditions with regard to the preoperative visus, the state of the lens, the range of the detachment, the age of the patients or the kind and the number of the retinal lesions influenced the choice of the operative method.

Following factors had significant influence on successful anatomic results after one Operation:
- Preoperative visus
- Low number of retinal lesions
- Experience of the surgeon
It was possible to collect data to the visus one year postoperative of 408 eyes. The visus was on average 0,47 log(MAR).
Following factors were significantly associated to a good visus one year postoperatively:

- Low severity of the Ablatio
- Young patient age
- Existence of a Horseshoe foramen
- Preoperative visus
- Existence of only one retinal hole
- Plomb or ppV with gas as operation method
The data of this work show, that the treatment of the rhegmatogenous retinal detachment with modern techniques and concepts offers good results, but still there are not all problems solved.
Actual trends in the literature for detachment surgery are confirmed.
