Abstract (German)
In Studien wurde ein gehäuftes Auftreten von bikuspiden Aortenklappen mit Aortenaneurysmata sowie mit Koronarektasien beobachtet. Sowohl das gehäufte Auftreten von Koronarektasien im Rahmen von Autoimmunerkrankungen und in Verbindung mit verschiedenen HLA II-Typen, als auch eine familiäre Häufung machen eine genetische Komponente von Koronarektasien wahrscheinlich. Die Pathogenese und Genetik dieser Erkrankung sind jedoch noch nicht hinlänglich bekannt. Neben dem gehäuften gemeinsamen Auftreten sprechen die verwandte Entwicklung von Aortenklappe, Aorta ascendens und Koronararterien in der Embryonalperiode sowie Gemeinsamkeiten in der Pathogenese für einen gemeinsamen genetischen Hintergrund von bikuspider Aortenklappe, Aortenaneurysmata und Koronarektasien. Matrix-Metalloproteasen, Renin-Angiotensin-Aldosteron-System und TGFβ-Signalweg sowie die für eNOS und ACE kodierenden Gene stellen überlappende Mechanismen dar. Ziel dieser Arbeit war es daher zu prüfen, ob SNPs, für die bereits eine Assoziation mit der bikuspiden Klappe oder dem thorakalen oder abdominalen Aortenaneurysma gefunden wurde, auch für Koronarektasien kodieren könnten.
Zu diesem Zweck wurden 337 Patienten mit und 1085 ohne Koronarektasien genotypisiert. Alle eingeschlossenen Patienten stammten entweder aus dem unselektierten Beobachtungsregister GoKard oder aus der Regensburger Herzinfarkt-Familienstudie (HIFAM), ein Register mit dem Einschlusskriterium auffälliger Familienanamnesen im Hinblick auf Myokardinfarkte. Eingeschlossen wurden Patienten, die bereits eine Herzkatheteruntersuchung erhalten hatten und von denen eine Blutprobe zur Genotypisierung vorhanden war. Nach systematischer Durchsicht der Bilder der Herzkatheteruntersuchung durch zwei erfahrene Untersucher wurden die Patienten in eine Fallgruppe mit und eine Kontrollgruppe ohne Koronarektasien eingeteilt.
Zur Genotypisierung wurden durch eine systematische Literaturrecherche in der PubMed-Bibliothek 22 SNPs ausgewählt, für die sich bereits eine Assoziation mit dem thorakalen oder abdominalen Aortenaneurysma, mit der bikuspiden Aortenklappe sowie zum Teil auch mit bikuspider Aortenklappe und assoziiertem Aneurysma der Aorta ascendens gezeigt hatte. Es wurden 7 SNPs auf dem FBN1-Gen, je zwei SNPs auf TGFBR2, CDKN2B-AS1 und NOS3 sowie je ein SNP auf den Genen MMP-2, TGFBR1, DAB2IP, LRP1, AGTR1, MTHFR, ZNF385D, MMP-9 und CNTN3 ausgewählt.
Es wurden drei statistische Auswertungen durchgeführt: Gesamte Fall- und Kontrollgruppe, gesamte Fall- und GoKard-Kontrollgruppe sowie HIFAM-Fall und GoKard-Kontrollgruppe.
Für insgesamt zehn der 22 untersuchten SNPs wurden teilweise signifikante Werte gefunden: Für vier der sieben untersuchten SNPs auf FBN1 (rs1036477, rs2118181, rs625034 und rs1036476), jeweils einen SNP auf TGFBR1 (rs1626340) und TGFBR2 (rs4522809), sowie für den jeweils untersuchten SNP auf LRP1 (rs1466535), AGTR1 (rs5186), MTHFR (rs1801133) und MMP-9 (rs2274755).
Keiner der untersuchten SNPs war in allen durchgeführten Auswertungen signifikant, und zum Teil zeigten sich inkonsistente Ergebnisse. Die Interpretation wurde zudem durch geringe Fallzahlen und teilweise durch Abweichungen der Genotypenverteilung vom Hardy-Weinberg-Equilibrium erschwert. Insbesondere für FBN1 ist jedoch eine Assoziation nicht auszuschließen. Zwar konnte der in der Assoziationstestung vermutete Effekt des jeweils seltenen Allels anhand der Genotypenverteilung nicht immer nachvollzogen werden. Dies könnte jedoch auf die zum Teil sehr geringe Anzahl des homozygoten Genotyps und die damit verbundene geringere Trennschärfe zurückzuführen sein.
Insgesamt konnte eine Assoziation der signifikant getesteten SNPs nicht bewiesen, aber größten Teils auch nicht eindeutig widerlegt werden.
Die Bedeutung von Koronarektasien für die klinische Praxis ist nicht eindeutig definiert. Trotz der allgemein guten Prognose einer reinen Koronarektasie ohne begleitende obstruktive koronare Herzerkrankung sollte die Erkrankung aufgrund der erhöhten Raten von Angina pectoris und vorangegangener Myokardinfarkte Beachtung finden. Das genauere Verständnis des genetischen Hintergrunds könnte den Umgang mit Koronarektasien im Hinblick auf Behandlungsstrategien und Prognose erleichtern. Weiterführende Untersuchungen mit größeren Fallzahlen und Subgruppenanalysen könnten sich daher als lohnenswert erweisen.

In studies, an assoication between bicuspid aortic valves and aortic aneurysms as well as coronary artery ectasia has been observed. Both the occurrence of coronary artery ectasia in the context of autoimmune diseases and in connection with different HLA II types, as well as a familial accumulation, make a genetic component of coronary artery ectasia likely. However, the pathogenesis and the genetic background of this disease are not yet fully understood. In addition to the co-occurrence, the commonalities in the embryonic development of aortic valve, ascending aorta and coronary arteries, as well as commonalities in pathogenesis, argue for an overlapping genetic background of bicuspid aortic valve, aortic aneurysm and coronary artery ectasia. Matrix metalloproteases, renin-angiotensin-aldosterone system, and TGFβ signaling pathway as well as genes encoding eNOS and ACE represent overlapping mechanisms. Therefore, the aim of this work was to examine whether SNPs already found to be associated with bicuspid valve or thoracic or abdominal aortic aneurysm might also be associated with coronary artery ectasia.
For this purpose, genotyping of 337 patients with and of 1085 patients without coronary artery ectasia was performed. All included patients were either part of the unselected observational registry GoKard or of the Regensburg Family Study of Myocardial Infarction (HIFAM), a registry for families with higher incidence of myocardial infarction. Patients who had already received cardiac catheterization and from whom a blood sample for genotyping was available were included. After systematic re-assessment of cardiac catheterization images by two experienced investigators, patients were assigned a case group with and a control group without coronary artery ectasia.
A systematic literature search in the PubMed library was conducted in order to select SNPs for which an association with thoracic or abdominal aortic aneurysm, bicuspid aortic valve, and, in some cases, bicuspid aortic valve and associated ascending aortic aneurysm had already been established. 22 SNPs were chosen for genotyping. Seven SNPs on FBN1, two SNPs each on TGFBR2, CDKN2B-AS1, and NOS3, and one SNP each on the MMP-2, TGFBR1, DAB2IP, LRP1, AGTR1, MTHFR, ZNF385D, MMP-9, and CNTN3 genes were selected.
Three statistical analyses were performed: Total Case and Control Group, Total Case and GoCard Control Group, and HIFAM Case and GoCard Control Group.
Partially significant results were found for a total of ten of the 22 SNPs: For four of the seven examined SNPs on FBN1 (rs1036477, rs2118181, rs625034, and rs1036476), one SNP each on TGFBR1 (rs1626340) and TGFBR2 (rs4522809), as well as for the examined SNP on LRP1 (rs1466535), AGTR1 (rs5186), MTHFR (rs1801133), and MMP-9 (rs2274755).
None of the examined SNPs showed significant results in all of the performed analyses and, in some cases, inconsistent results were observed. Interpretation was further complicated by small case numbers and by occasional deviations of the genotype distribution from the Hardy-Weinberg equilibrium. However, especially for FBN1, an association cannot be excluded. The effect of the respective rare allele assumed in the association test could not always be reconstructed on the basis of the genotype distribution. However, this migth be due to the sometimes very low number of the homozygous genotype and the consequently lower discriminatory power.
Overall, an association of the SNPs wtih significant test results could not be proven, but mostly also not be clearly refuted.
The relevance of coronary artery ectasia to clinical practice is not yet clearly defined. Despite the overall good prognosis of pure coronary artery ectasia without accompanying obstructive coronary artery disease, the disease should not be neglected because of the increased rates of angina and previous myocardial infarction. A better understanding of the genetic background may facilitate the clinical management of coronary artery ectasia regarding treatment strategies and prognosis. Further studies with larger case numbers and subgroup analyses might therefore be worthwhile.
