Abstract (German)
Der Faktor XIII oder s.g. fibrinstabilisierender Faktor ist als letztes Glied in der Gerinnungskaskade zuständig für die Vernetzung und Stabilisierung des Fibrinthrombus und spielt dadurch eine wichtige Rolle in der Blutgerinnung.
Im Gegensatz zum angeborenen Faktor XIII Mangel ist die Inzidenz eines erworbenen Faktor XIII Mangels wesentlich höher. So kann es z.B. bei großen operativen Eingriffen in der Allgemein- und Abdominalchirurgie sowie Herzthoraxchirurgie intra- und postoperativ zu einem Verbrauch von Faktor XIII im Rahmen der Blutstillung und Wundheilung kommen.
Im Gegensatz zum angeborenen Faktor XIII Mangel ist die klinische Relevanz eines sekundären Faktor XIII Mangels unklar und in der Literatur ist die Indikationen zur Substitution eines Faktor XIII Mangels nicht klar definiert.
Im Rahmen dieser medizinischen Doktorarbeit wurden die Inzidenz, die Diagnostik und die Behandlung des Faktor XIII Mangels am Universitätsklinikum Regensburg analysiert und die klinische Relevanz des Faktor XIII Mangels untersucht werden. Hierbei wurde in einem definierten Zeitraum (01.01. – 31.12.2017) ermittelt, wie viele Faktor XIII Bestimmungen im Gerinnungslabor des Instituts für Klinische Chemie und Laboratoriumsmedizin angefordert wurden und in welchen klinischen Situationen eine Faktor XIII Substitution erfolgte. Das retrospektive Forschungsvorhaben fußt dabei auf der Analyse von ärztlichen Dokumenten, Laborwerten und Akten von Patienten am Universitätsklinikum Regensburg, bei denen eine Faktor XIII Bestimmung im oben angegebenen Zeitraum durchgeführt wurde.
Im Jahr 2017 wurden die Faktor XIII Bestimmungen einmal pro Woche durchgeführt. Seit 2019 gibt es im Gerinnungslabor eine neue Laborordnung für die Faktor XIII Analyse, wodurch die Verfügbarkeit dieser Bestimmung auf täglicher Basis erhöht wurde. Ein weiteres Ziel dieser Studie war es zu untersuchen, ob Unterschiede bei der Verabreichung von Faktor XIII Konzentraten durch die Einführung der neuen Laborordnung für die Faktor XIII Analyse im Jahr 2019 gegenüber 2017 festgestellt wurden.
Zusammenfassend lässt sich feststellen, dass mit der Einführung der neuen Laborordnung, die Anzahl der Faktor XIII Bestimmungen sowie die Verwendung von plasmatischen Faktor XIII Präparaten am Universitätsklinikum Regensburg im Jahr 2019 deutlich gestiegen sind. Die höchste Inzidenz von schwerem und mittelschwerem Faktor XIII Mangel am Universitätsklinikum Regensburg war im Bereich der Intensivmedizin zu verzeichnen, insbesondere im Zusammenhang mit der ECMO-Therapie. Bei den meisten Patienten mit verminderter Faktor XIII Aktivität wurden schwere Blutungsereignisse mit erhöhtem Bedarf an Bluttransfusionen dokumentiert, die in den meisten Fällen mit der ECMO-Therapie in Zusammenhang standen. Die Zunahme der Anwendung der Faktor XIII Präparaten wurde hauptsächlich bei den Patienten mit schwerem oder mittelschwerem Faktor XIII Mangel beobachtet, während die Anwendung von Fibrogammin® bei den Patienten mit normaler Faktor XIII Aktivität im Vergleich zu 2017 leicht zurückging. Die Zunahme der Faktor XIII Ersatztherapie im Jahr 2019 lässt sich durch das neu eingeführte standardisierte Protokoll zur Gerinnungsdiagnostik während der ECMO-Therapie erklären. Da es bisher keine einheitliche Empfehlung für die Faktor XIII Substitutionstherapie gibt, spekulieren wir, dass Kliniker auch aufgrund von klinischen Befunden, eine Substitutionstherapie mit Fibrogammin® eingeleitet haben könnten. Diese Vermutung wird durch die Tatsache untermauert, dass alle mit Fibrogammin® behandelten Patienten relevante blutungsbedingte Komplikationen aufwiesen, während nur ein Drittel der Patienten mit nachgewiesenem ausgeprägtem Faktor XIII Mangel eine Fibrogammin®-Ersatztherapie erhielt. Weitere prospektive randomisierte Studien zur Inzidenz, optimalen Überwachung und zu den Behandlungszielen des erworbenen Faktor XIII Mangels sind von entscheidender Bedeutung, um effiziente Algorithmen für das Management von Blutungskomplikationen zu etablieren.

Factor XIII, or so-called fibrin-stabilizing factor, is the last link in the coagulation cascade and is responsible for the cross-linking and stabilization of the fibrin thrombus, thus playing an important role in blood coagulation.
In contrast to congenital factor XIII deficiency, the incidence of acquired factor XIII deficiency is much higher. For example, during major surgical procedures in general and abdominal surgery as well as cardiothoracic surgery, hemostasis and wound healing can lead to intraoperative and postoperative consumption of factor XIII.
In contrast to congenital factor XIII deficiency, the clinical relevance of secondary factor XIII deficiency is unclear and the indications for substitution of factor XIII deficiency are not clearly defined in the medical literature.
Therefore the incidence, diagnosis and treatment of factor XIII deficiency at the University Hospital Regensburg were analysed and the clinical relevance of factor XIII deficiency was investigated in this study. For this purpose, in a defined period (01.01. - 31.12.2017), it was determined how many factor XIII activity assays were requested in the coagulation laboratory of the Institute of Clinical Chemistry and Laboratory Medicine and in which clinical situations a factor XIII substitution took place. The retrospective research project is based on the analysis of medical documents, laboratory values and files of patients at the University Hospital Regensburg in whom a factor XIII determination was performed in the above-mentioned period.
In 2017, factor XIII assays were performed once a week. Since 2019, a new laboratory order for factor XIII analysis was introduced in the coagulation laboratory, which increased the availability of this assay on a daily basis. Therefore another objective of this study was to investigate whether differences in the administration of plasmatic factor XIII concentrates were observed due to the introduction of the new laboratory order for factor XIII analysis in 2019 compared with 2017.
In summary, with the introduction of the new laboratory order, the number of factor XIII activity assays as well as the administration of plasmatic factor XIII concentrates at Regensburg University Hospital increased significantly in 2019. The highest incidence of severe and moderate factor XIII deficiency at the University Hospital Regensburg was in the intensive care setting, especially in association with ECMO therapy. Most patients with decreased factor XIII activity were documented to have severe bleeding events, which in most cases were related to ECMO therapy. The increase in the use of factor XIII concentrates was mainly observed in the patients with severe or moderate FXIII deficiency, while the use of Fibrogammin® slightly decreased in the patients with normal factor XIII activity compared to 2017. The increase in factor XIII replacement therapy in 2019 can be explained by the newly introduced standardised protocol for coagulation diagnostics during ECMO therapy. As there is no standard recommendation for factor XIII replacement therapy to date, we speculate that clinicians may have also initiated Fibrogammin® replacement therapy based on clinical findings. This speculation is supported by the fact that all patients treated with Fibrogammin® had relevant bleeding-related complications, while only one-third of patients with documented FXIII deficiency received Fibrogammin® replacement therapy. Further prospective randomised studies on the incidence, optimal monitoring and treatment targets of acquired factor XIII deficiency are crucial to establish efficient algorithms for the management of bleeding complications.
