Abstract (German)
HINTERGRUND: Diese Arbeit wurde durchgeführt, um die vermeintlich erhöhte Inzidenz an neu aufgetretenen postoperativen intrakardialen Blockierungen nach einem chirurgischen Aortenklappenersatz mit der ballonexpandierbaren nahtlosen Edwards-Intuity Elite Prothese im Vergleich mit der konventionellen Edwards-Perimount Magna Ease Prothese zu untersuchen. HINTERGRUND:
Diese Arbeit wurde durchgeführt, um die vermeintlich erhöhte Inzidenz an neu aufgetretenen postoperativen intrakardialen Blockierungen nach einem chirurgischen Aortenklappenersatz mit der ballonexpandierbaren nahtlosen Edwards-Intuity Elite Prothese im Vergleich mit der konventionellen Edwards-Perimount Magna Ease Prothese zu untersuchen.
METHODEN:
Zwischen Mai 2014 und Mai 2016 wurden 100 konsekutive Patienten, bei denen eine nahtlose ballonexpandierbare Edwards Intuity Elite Aortenklappenprothese implantiert wurde, und 100 konsekutive Patienten, bei denen eine konventionell genähte Klappenprothese Perimount Magna Ease implantiert wurde, retrospektiv miteinander verglichen. Dabei wurde der Schwerpunkt auf die neu aufgetretenen intrakardialen Blockierungen perioperativ und nach einem Nachbeobachtungszeitraum von einem Jahr gesetzt.
ERGEBNISSE:
Die Inzidenz des neu aufgetretenen Linksschenkelblocks war direkt nach der Operation, zum Zeitpunkt der Entlassung aus dem Krankenhaus und nach einem Nachbeobachtungszeitraum von einem Jahr 41,8%, 36,3% und 18,7% in der Intuity Gruppe vs. 6,3%, 4,2% and 1,1% in der Magna Gruppe. Die Inzidenz des neu aufgetretenen AV-Block III war 5,4%, 10,8% and 10,8% vs. 5.3%, 4.2% and 4.2%. Die Inzidenz des neu aufgetretenen Rechtsschenkelblocks war in beiden Gruppen gleich niedrig und blieb im Verlauf stabil (2,2% vs. 2,3%).
Die transvalvulären Gradienten über der Intuity Prothese waren niedriger, als über der konventionell genähten Magna Prothese (Pmean 8,6 ± 3,6 mmHg vs. Pmean 12,3 ± 4,0 mmHg). In der Intuity Gruppe traten 2 paravalvuläre Leckagen größer Grad 1 (2,1%) vs. 0% in der Magna Gruppe auf. Die 30-Tages Mortalität war zwischen den Gruppen nicht signifikant unterschiedlich (3,0% vs. 3,1%).
SCHLUSSFOLGERUNG:
Die Implantation der nahtlosen ballonexpandierbaren Aortenklappenprothese Edwards Intuity Elite ist, verglichen mit der konventionell genähten Klappenprothese Perimount Magna Ease, mit einer erhöhten Inzidenz eines postoperativen AV-Blocks III und vor allem eines postoperativen Linksschenkelblocks verbunden.
Andererseits zeigt die Intuity Prothese niedrigere transvalvuläre Gradienten als die Magna Prothese, was auf eine bessere transvalvuläre Hämodynamik hindeutet.
Dementsprechend muss vor Implantation einer Intuity Prothese das Risiko einer erhöhten Inzidenz intrakardialer Blockierungen gegen den Vorteil und die Notwendigkeit einer besseren postoperativen Hämodynamik individuell abgewogen werden.

OBJECTIVES:
The rate of postoperative conduction abnormalities after implantation of rapid-deployment aortic valves seems to be increased. We were interested in the incidence of new postoperative left bundle branch block, right bundle branch block and atrioventricular block III after the implantation of the Edwards Intuity Elite prosthesis.
METHODS:
Between May 2014 and May 2016, 100 consecutive patients, undergoing aortic valve replacement with the Edwards Intuity Elite prosthesis, were retrospectively compared with 100 consecutive patients, undergoing aortic valve replacement with the Edwards Perimount Magna Ease prosthesis. The occurrence of new postoperative conduction abnormalities perioperatively and after a 1-year follow-up were evaluated.
RESULTS:
Right postoperatively, at the time of hospital discharge and after a 1-year follow-up 41.8%, 36.3% and 18.7% of the patients in the Intuity group developed a new left bundle branch block and 5.4%, 10.8% and 10.8% a new atrioventricular block III. Correspondingly 6.3%, 4.2% and 1.1% of the patients in the Magna group developed a new left bundle branch block and 5.3%, 4.2% and 4.2% a new atrioventricular block III. The rate of a new postoperative right bundle branch block was low in both groups and stable over time (2.2% vs. 2.3%).
The postoperative mean transvalvular pressure gradient across the Intuity prosthesis was 8.6 ± 3.6 mmHg vs. 12.3 ± 4.0 across the Magna prosthesis. The occurrence of paravalvular leakage more than trivial was 2.1% in the Intuity group vs. 0% in the Magna group and the hospital mortality 3.1% vs. 3.0%.
CONCLUSIONS:
The incidences of new postoperative left bundle branch block and atrioventricular block III were considerably increased after Implantation of the Edwards Intuity Elite rapid- deployment aortic valve prosthesis, compared with the Edwards Perimount Magna Ease aortic valve prosthesis. However, the Intuity prosthesis showed superior hemodynamic performance compared with the Magna Prostesis.
Accordingly, before implantation of an Intuity prosthesis we recommend to weigh the risk of postoperative conduction abnormalities against the benefit of better hemodynamic performance for each individual patient.
