Das Ziel der Dissertation war es, festzustellen, ob zusätzlich zur radikalen Hysterektomie die adjuvante Radio(chemo)therapie im Frühstadium des Zervixkarzinoms (FIGO IB-IIA) einen Überlebensvorteil birgt. Dafür gingen wir folgendermaßen vor:
Seit 2002 sind die Krankenhäuser im Raum Niederbayern und Oberpfalz verpflichtet, sämtliche Daten zu Malignom-Patienten an das Tumorzentrum weiterzuleiten. Aus diesen archivierten schriftlichen Meldungen wurde ein Datensatz gebildet, aus dem sich die vorliegende retrospektive Kohortenstudie speist. Von insgesamt 1.613 Patientinnen mit Zervixkarzinom im Erhebungszeitraum 2002-2015 entsprachen 522 Patientinnen dem eingeschlossenen Tumorstadium FIGO IB-IIA. Davon wiederum konnten 462 Patientinnen ins Studienkollektiv eingeschlossen werden (z.B. Ausschluss von seltenen histologischen Subtypen, oder bei ungenügender medizinischer Dokumentation).
Anschließend wurden Subgruppen zur statistischen Auswertung gebildet: Wir stratifizierten die Patientinnen in Orientierung an der S3-Leitlinie nach high risk-Faktoren (postoperative R1-Situation und befallene Lymphknoten) und intermediate risk-Faktoren (Veneninvasion, Lymphgefäßinvasion, Tumorgröße >4 cm, Grading G3) in Gruppen mit unterschiedlich hoher Risikokonstellation: Niedrig-Risiko-Gruppe ohne vorliegende Risikofaktoren, Mittleres-Risiko-Gruppe mit einem oder zwei intermediate risk-Faktor und Hochrisiko-Gruppe mit entweder mindestens einem high risk-Faktor und/oder mindestens drei intermediate risk-Faktoren. Innerhalb dieser Risikogruppen wurden Regressionsanalysen zum Gesamtüberleben und dem krankheitsbezogenen Überleben durchgeführt.
In keiner der drei Subgruppen ließ sich ein klarer Vorteil für die adjuvante Therapie nach Operation feststellen: In der low risk-Gruppe zeigte sich sogar ein signifikant verringertes rezidivfreies Überleben im adjuvanten Therapie-Arm. Für die intermediate risk-Gruppe zeigte sich beim RFS eine Verbesserung für die Patientinnen, die nach der Hysterektomie eine Radiochemotherapie erhielten. In der high risk-Gruppe zeigte sich dieser Effekt nur tendenziell durch verringerte Hazard Ratios im OS und RFS für eine adjuvante RCT (statistisch nicht signifikant).
Anhand unserer Ergebnisse sollten Frauen mit low risk-Zervixkarzinom im Frühstadium keine adjuvante Therapie erhalten. In intermediate- und high risk-Fällen scheint für die adjuvante Therapie ein (statistisch nicht signifikanter) Vorteil zu existieren.

Translation of the abstract (English)

The goal was to determine whether there is a benefit for adjuvant radio(chemo)therapy in early cervical cancer (FIGO IB-IIA).
We were able to form a cohort out of the cancer registry of the Tumorzentrum Regensburg collecting data from the regions Lower Bavaria and Upper Palatinate since 2002. Out of all the registered 1,613 cervical carcinoma patients from 2002-2015, 522 patients belonged to FIGO stadium IB-IIA. Of those, 462 met our inclusion criteria (i.e. excluding rare histologic types or insufficient documentation).
We proceeded by forming subgroups for statistical analysis, stratifying the patients in accordance with the German s3 guideline into a low, intermediate and high risk-group. Patients in the low risk-group didn’t show any risk factors, patients in the intermediate risk-group had one or two intermediate risk-factors and patients in the high risk-group showed at least one high risk-factor or three or more intermediate risk-factors. Intermediate risk-factors were tumor size >4cm, grading G3, lymphatic and/or vascular invasion. High risk-factors were positive lymph nodes or positive surgical margins after hysterectomy. Within these subgroups, we conducted multivariable regression analyses for overall survival and recurrence-free survival.
None of our subgroups showed a clear benefit for adjuvant therapy after hysterectomy: Among the low risk-group, there was even a significantly lower recurrence-free survival for those with adjuvant therapy.
For the intermediate risk-group, there was a benefit for recurrence-free survival for patients with an adjuvant radiochemotherapy.
Among the high risk-group, this effect was indicated through lower hazard ratios for adjuvant radiochemotherapy in OS and RFS; although not statistically significant.
In conclusion, patients with an early-stage low risk-cervical carcinoma should not receive an adjuvant therapy after hysterectomy. Among the intermediate and high risk-cases, there seems to be a benefit for adjuvant therapiy; albeit not statistically significant in our cohort.
