Hintergrund: Postoperatives Vorhofflimmern (POVHF) ist eine der häufigsten Komplikationen nach herzchirurgischen Eingriffen. Es ist verbunden mit einem erhöhten Morbiditäts- und Mortalitätsrisiko, mit einem verlängerten Krankenhausaufenthalt sowie höheren Kosten für die postoperative Versorgung. Die Zusammenhänge zwischen schwergradiger schlafbezogener Atmungsstörungen (SBAS) und POVHF oder zentraler Schlafapnoe (ZSA) und POHVF wurden bisher nicht ausreichend untersucht. Ziel dieser Sub-Analyse ist es, die Inzidenz von neu auftretendem POVHF nach elektiver aortokoronarer Bypass-Operation (ACB-OP) bei Patienten mit und ohne schweren SBAS mit Unterscheidung zwischen obstruktiver Schlafapnoe (OSA) und ZSA zu untersuchen.
Methoden: Bei den 415 Patienten der CONSIDER AF Sub-Analysen-Kohorte (NCT: 02877745) wird in der Klinik für Herz-Thorax-Chirurgie des UKR eine Polygraphie zur Diagnose und Bestimmung des Schweregrads einer SBAS in der präoperativen Nacht durchgeführt. Schwergradige SBAS sind definiert als ein Apnoe-Hypopnoe-Index ≥ 30/h. Die Diagnose schwergradige OSA wird gestellt, wenn zudem ≥ 50 % aller Apnoen obstruktiv sind, bei <50 % wird die Diagnose einer schwergradigen ZSA gestellt. Die Inzidenz von neu aufgetretenem POVHF innerhalb des postoperativen Krankenhausaufenthalts wird mittels 12-Kanal-Ruhe-EKGs ermittelt. Früh-einsetzendes POVHF ist definiert als mindestens eine Episode von POVHF innerhalb des Krankenhausaufenthalts. 55 Patienten mit Vorhofflimmern in der präoperativen Vorgeschichte werden für diese Analyse ausgeschlossen.
Ergebnisse: Bei 79 % der Patienten liegt keine oder eine milde/moderate und bei 21 % eine schwergradige SBAS vor, davon sind 9 % OSA und 12 % ZSA. Die Patienten, die sich einer ACB-OP unterziehen und eine schwere SBAS aufweisen, sind signifikant häufiger von früh-einsetzendem POVHF betroffen als Patienten ohne schwere SBAS (keine oder milde/moderate SBAS vs. schwere OSA vs. schwere ZSA: 15 % vs. 29 % vs. 30 %, p=0,033). 22 % der Patienten mit früh-einsetzendem POVHF haben innerhalb der ersten 60 Tage postoperativ mindestens ein Vorhofflimmer-Rezidiv. Zudem sind schwere SBAS zusammen mit fortgeschrittenem Alter und Diabetes mellitus nach Anpassung für bekannte Störfaktoren signifikant mit früh-einsetzendem POVHF assoziiert. Die Patienten mit früh-einsetzendem POVHF leiden während des postoperativen Krankenhausaufenthaltes häufiger an akuter Herzinsuffizienz, postoperativer Hypoxämie, akuter Nierenschädigung Stadium 2-3 und an einem Delir. Zudem ist die Dauer des Krankenhausaufenthalts im Median um 5 Tage, der Aufenthalt auf Intensivstation bzw. IMC im Median um 2 Tage länger als bei Patienten ohne POVHF. Nur 8 % der Patienten mit POVHF konvertieren bis zur Entlassung spontan zurück in den Sinusrhythmus und bei 12 % wird im Entlassungsbrief keine Therapie erwähnt. Zum Zeitpunkt der Entlassung erhalten 37 % und 60 Tage postoperativ 45 % der betroffenen Patienten eine Antikoagulation.
Schlussfolgerung: Alle Patienten, welche sich einer elektiven ACB-OP unterziehen, sollten präoperativ auf ihr SBAS-Risiko untersucht werden und der Herzrhythmus sollte postoperativ regelmäßig evaluiert werden. Ob SBAS zur Entwicklung von POVHF nach einer Herzoperation beiträgt, die durch eine angemessene Behandlung, z.B. CPAP, gelindert werden kann, bedarf weiterer gründlicher Untersuchungen.

Background: Postoperative atrial fibrillation (POAF) is one of the most common complications after cardiac surgery. It is associated with increased risk of morbidity and mortality, prolonged hospital stays, and higher costs of postoperative care. The associations between severe sleep-disordered breathing (SDB) and postoperative atrial fibrillation or central sleep apnea (CSA) and POAF have not been adequately studied. The aim of this sub-analysis is to investigate the incidence of new-onset POVHF after elective aortocoronary bypass (ACB) surgery in patients with and without severe SDB with differentiation between obstructive sleep apnea (OSA) and CSA.
Methods: In the 415 patients of the CONSIDER AF sub-analysis cohort (NCT: 02877745), polygraphy is performed at the Department of Cardiothoracic Surgery of the UKR to diagnose and determine the severity of SDB on the preoperative night. Severe SDB is defined as an apnea-hypopnea index ≥ 30/h. If in addition ≥ 50% of all apneas are obstructive, a diagnosis of severe OSA is made. If <50% of all apneas are obstructive, a diagnosis of severe CSA is made. The incidence of new-onset POAF within the postoperative hospital stay is determined by 12-lead resting ECGs. Early-onset POAF is defined as at least one episode of POAF within the hospital stay. 55 patients with history of atrial fibrillation (AF) preoperatively are excluded for this analysis.
Results: No or mild/moderate SDB is present in 79% of patients and severe SDB in 21%, of which 9% are OSA and 12% are CSA. The patients who undergo ACB surgery and have severe SDB are significantly more likely to have early-onset POAF than patients without severe SDB (no or mild/moderate SDB vs. severe OSA vs. severe CSA: 15% vs. 29% vs. 30%, p=0.033). 22% of patients with early-onset POAF have at least one AF recurrence within the first 60 days postoperatively. In addition, severe SDB along with advanced age and diabetes mellitus are significantly associated with early-onset POAF after adjustment for known confounders. The patients with early-onset POAF are more likely to suffer from acute heart failure, postoperative hypoxemia, acute kidney injury stage 2-3, and delirium during the postoperative hospital stay. In addition, the median length of hospital stay is 5 days longer, and the median length of stay at the intensive care unit or IMC is 2 days longer than in patients without POAF. Only 8% of patients with POAF spontaneously convert back to sinus rhythm by the time of discharge, and no therapy is mentioned in the discharge letter for 12%. At the time of discharge, 37% and 60 days postoperatively 45% of affected patients receive anticoagulation.
Conclusion: All patients undergoing elective ACB surgery should be evaluated preoperatively for SBAS risk, and cardiac rhythm should be evaluated regularly postoperatively. Whether SDB contributes to the development of POAF after cardiac surgery that can be alleviated by appropriate treatment, such as continuous positive airway pressure (CPAP), requires further thorough investigation.
