Abstract (German)
Distale Radiusfrakturen sind die häufigsten Frakturen im Kindes- und Jugendalter (Randsborg et al. 2013). Dabei begünstigen Salter-Harris-Frakturen oder Verletzungen fugennaher Gefäße hemmende Wachstumsstörungen, welche im Verlauf zu Komplikationen wie Schmerzen und Bewegungseinschränkungen führen können (Cannata et al. 2003). Als weiterer Risikofaktor werden operative Behandlungsverfahren wie das Einbringen eines Kirschner-Drahtes durch die Epiphysenfuge diskutiert. Die etablierten Verfahren zur Diagnostik von Wachstumsstörungen sind Röntgen und CT. Da das kindliche Gewebe im Vergleich zum Erwachsenen eine ca. zehnfach höhere Strahlensensibilität (Brenner et al. 2001) (Hall 2002) aufweist, ist die Entwicklung eines alternativen Diagnostikverfahrens dringend sinnvoll. Ziel der vorliegenden Arbeit ist es, die Sonografie als strahlenfreie, schnell und einfach im klinischen Alltag anwendbare Screeningmethode für Wachstumsstörungen des distalen Radius zu testen.
Posttraumatische hemmende Wachstumsstörungen des distalen Radius fallen durch eine veränderte Längendifferenz zwischen Radius und Ulna auf und stellen sich als stark ausgeprägte Ulna-Plusvarianten dar. Um diesen potenziellen posttraumatischen Unterschied ermitteln zu können, wurde die Längendifferenz zu zwei Zeitpunkten erhoben. Die erste Vermessung erfolgte mittels der Hafner Methode (Hafner et al. 1989) auf Röntgenbildern, welche zum Zeitpunkt der Fraktur entstanden. Dabei wurde der Abstand der Unterarmknochen zwischen den distalsten Punkten der distalen Metaphysen von Radius und Ulna ermittelt. Zum Zeitpunkt des Follow-Ups
erfolgte die Vermessung mittels der selbst entwickelten sonografischen Methode. Hierfür wurde der Ultraschallkopf auf dem pronierten Unterarm in der Sagittalebene von der distalen Radiusfuge zu der distalen Ulnafuge geschwenkt und ebenfalls zwischen den distalsten Punkten der Metaphysen vermessen. Anschließend wurde die Differenz beider Werte gebildet.
Insgesamt nahmen 126 Patienten mit Zustand nach traumatischer distaler Radiusfraktur am Follow-Up teil. Bei 2 Mädchen (15 und 17 Jahre) und 3 Jungen (15, 16 und 17 Jahre) war die Epiphysenfuge aufgrund des fortgeschrittenen Wachstums in der Sonografie nicht darstellbar. Von den restlichen 121 Patienten entsprachen 4,0 % Salter-Harris-Frakturen Typ I, 8,0 % Salter-Harris-Frakturen Typ II, 74,4 % metaphysären und 13,6 % diametaphysären Frakturen. 64,5 % der Patienten waren männlich, 35,5 % weiblich. Zum Zeitpunkt des Röntgenbildes waren die Patienten im Schnitt 8,1 Jahre alt, das Follow-Up erfolgte durchschnittlich 3 Jahre später. Die mittlere Längendifferenz zwischen Radius und Ulna lag für die sonografische Methode bei -6,7 mm, für die per Röntgen ermittelten Werte bei -2,4 mm. Die Differenz der beiden Mittelwerte von 4,3 mm ist hoch signifikant (t = -21,4, p<0,001).
Aufgrund des hoch signifikanten Unterschiedes der Mittelwerte von den per
Röntgen und Sonografie erhobenen Längendifferenzen eignet sich deren Vergleich nicht als Diagnoseparameter einer Wachstumsstörung. Die Hauptgründe für die große Abweichung der Werte sind der zeitliche Abstand der Aufnahmen, die potenziellen posttraumatischen Veränderungen und die unterschiedlichen Stellungen des Unterarms während der Bildaufnahme. Die per Sonografie ermittelten Längendifferenzen von Radius und Ulna allein als Diagnosekriterium betrachtend, ergab sich kein Hinweis auf eine Wachstumsstörung des distalen Radius im vorliegenden
Kollektiv. Dies ist hauptsächlich auf den Anteil fugenkreuzender Frakturen von nur 15 Patienten (12,0 %) zurückzuführen. In der Literatur sind 1,9 % (Davis und Green 1976) bis 4,4 % (Cannata et al. 2003) hemmende Wachstumsstörungen des distalen Radius bei Studienpopulationen beschrieben, welche ausschließlich aus Patienten mit Salter-Harris-Frakturen bestehen. Da sich bei den 121 untersuchten Patienten die Epiphysenfugen in der Sonografie offen darstellten und sich nur Ulna-Minusvarianten ergaben, kann das Vorliegen einer hemmenden Wachstumsstörung im untersuchten Patientenkollektiv mit großer Wahrscheinlichkeit ausgeschlossen werden.
Somit kann die selbstentwickelte sonografische Methode empfohlen werden, um mittels der Aussage über Ulna-Plus- oder Ulna-Minusvariante einerseits und der Beurteilung der distalen Epiphysenfugen von Radius und Ulna andererseits eine erste Aussage über das Vorliegen einer hemmenden Wachstumsstörung des distalen Radius zu treffen. Es bräuchte weitere Studien mit einem größeren Anteil an Salter-Harris-Frakturen, um Grenzwerte für pathologische Längendifferenzen für die sonografische Methode festzulegen und um beim Vorliegen einer Wachstumsstörung sicher aussagen zu können, ob die neue Methode diese zuverlässig nachweisen kann.

Fractures of the distal radius are the most common bone fractures in children and adolescents (Randsborg et al. 2013). Salter-Harris fractures or injuries to vessels close to the joint increase the risk of growth retardation, which can lead to complications such as pain and restricted mobility (Cannata et al. 2003). A further risk factor is surgery with the method of inserting a Kirschner wire through the growth plate. The established methods for diagnosing growth disturbances are X-ray and CT. Since the child's tissue is approximately ten times more sensitive to radiation (Brenner et al. 2001) (Hall 2002) compared to adults, the development of an alternative diagnostic procedure is urgently needed. The aim of this study is to test sonography as a radiation-free screening method for growth disturbances of the distal radius that can be used quickly and easily in everyday clinical practice.
Post-traumatic growth arrest of the distal radius is characterized by an altered difference in length between the radius and ulna and presents as a strongly expressed positive ulnar variance. To determine this potential post-traumatic difference, the length difference was measured at two different times. The first measurement was taken using the Hafner method (Hafner et al. 1989) on X-ray images of the time, when the fracture happened. In this method the length difference of the forearm bones was determined between the most distal points of the distal metaphyses of the radius and ulna. At the time of the follow-up the measurement was carried out using the sonographic, self-developed method. For this purpose, the ultrasound transducer was pivoted on the pronated forearm in the sagittal plane from the distal radius growth plate to the distal ulna one and the length difference was also measured between the most distal points of the metaphyses of the radius and ulna. The difference between the two values of the different measurements was then calculated.
A total of 126 patients with a traumatic distal radius fracture participated in the follow-up. In 2 girls (15 and 17 years) and 3 boys (15, 16 and 17 years), the growth plate could not be shown on sonography due to advanced growth. Of the remaining 121 patients, 4.0% were Salter-Harris type I fractures, 8.0% were Salter-Harris type II fractures, 74.4% were metaphyseal fractures and 13.6% were diametaphyseal fractures. 64.5% of the patients were male, 35.5% female. At the time of the X-ray, the patients were on average 8.1 years old and the follow-up was on average 3 years later. The length difference between the radius and ulna was on average -6.7 mm for the sonography, for the values determined by X-ray it was on average -2.4 mm. The difference between the two mean values of 4.3 mm is highly significant (t = -21.4, p<0.001).
Because of the highly significant difference in the mean values of the length differences determined by the two different methods, their comparison is not suitable as a diagnostic parameter for a growth arrest. The main reasons for the large deviation in the values are the time interval between the images, the potential post-traumatic changes and the different positions of the forearm while the image was taken. Considering only the length difference of the radius and ulna which was determined by sonography, there was no indication for a growth arrest of the distal radius in the present group. This is mainly due to the small proportion of Salter-Harris fractures (12.0%). In the literature there are 1.9% (Davis and Green 1976) to 4.4% (Cannata et al. 2003) of post-traumatic growth disturbances of the distal radius described, this being in study populations consisting only of patients with Salter-Harris fractures. Since the growth plates in the 121 patients examined were open on ultrasonography and only ulnar minus variants were found, the presence of an growth arrest in the patient population examined can be ruled out with a high degree of probability.
The self-developed sonographic method can therefore be recommended to make a first statement about the presence of a growth arrest of the distal radius by means of the statement about ulna plus or ulna minus variant on the one hand and the assessment of the distal growth plate of the radius and ulna on the other. Further studies with a larger proportion of Salter-Harris fractures would be required in order to define exact values for pathological length differences for the sonographic method and to be able to state with certainty whether the new method can reliably detect a growth arrest if it is present.
