The well-known side effects of contrast agent-induced nephropathy and nephrogenic systemic fibrosis by computed tomography (CT) and magnetic resonance angiography (MRA) restrict their use in daily practice. Therefore ultrasound has experienced a renaissance and colour-coded duplex sonography of vessels plays a very significant role in the diagnostics and therapy planning of vascular diseases in all applicable disciplines. Diagnostic algorithms are presented within the framework of carotid artery stenosis, peripheral arterial occlusive disease and abdominal aortic aneurysms which are therapy-oriented and effective for planning the further steps. The value of highly competent ultrasonic diagnostics reflects the high demands of specialist training. The delegation of ultrasound examination to non-medical personnel with specialist training for screening of high risk cardiovascular patients under supervision by qualified physicians depends on the importance of color-coded duplex sonography for the vascular diagnostics.
