Fostering the development of a successful and respected surgical researcher first requires an absolute devotion from the individual to achieve this goal, and second, enabling institutional structures. A formal and potentially useful mechanism to promote the careers of surgeon-scientists is to establish an institution of "experimental surgery'' that cooperates very closely with, or within, a department of surgery. However, in developing a successful experimental surgery programme, there are several factors that are crucial in determining the level and impact of research possible. Indeed, it can be argued that experimental surgical research tends to generate a mediocre impact on medicine; however, this is not an inevitable outcome, and in the age of translational medicine, can be rather extraordinarily powerful research. In this article the most important of these factors from my own experience will be discussed. In general, harnessing the positives that surgeon-scientists have to offer can be rewarding, but achieving exceptional success in this respect demands special guidance and support from structures such as institutions of experimental surgery.
