We report on a case of fatty liver during pregnancy in a 39-year-old primipara with a twin pregnancy of 30 weeks' gestation. Apart from a generalized pruritus and highly elevated liver enzymes the patient showed no clinical symptoms. Liver sonography could not detect the hyperechoic structures typical for fatty infiltration of liver tissue. Intensive care monitoring of the patient allowed a prolongation of pregnancy for 12 days. Caesarean section as well as liver biopsy were performed at 31 + 2 weeks of gestation. The histological finding confirmed acute fatty liver during pregnancy. Conclusion: Most case reports on fatty liver during pregnancy describe a highly acute event. Apart from those cases there are rare and atypical courses which make the correct diagnosis difficult, especially when imaging diagnostics fail to provide more evidence.
