Ein beginnendes Demenzsyndrom zeigt sich nicht nur durch leichte kognitive Beeinträchtigungen (Mild Cognitive Impariment), sondern durch Verhaltensauffälligkeiten und andere nicht-kognitive psychische Symptome besonders in der Frühphase. Dieser Symptomkomplex wird seit 2016 als Mild Behavioral Impairment (MBI) zusammengefasst.
Eine der noch ungelösten Schwierigkeiten bei MBI ist die Unsicherheit, inwiefern das Vorliegen derartiger Symptome beim individuellen Patienten für eine neurodegenerative Erkrankung indikativ ist oder eher auf eine andere Erkrankung, z.B. eine depressive Episode, hinweist oder welche der einzelnen nicht-kognitiven Symptome am meisten mit der späteren Konversion in eine manifeste Demenz korrelieren.
Zu diesem Zweck wurde die MBI-Checkliste entwickelt, welche es Forschungsgruppen und klinischen Fachpersonal erleichtern soll, ein MBI zu diagnostizieren.
Es handelt sich um einen strukturierten Fragebogen, mit dessen Hilfe die vorliegenden Symptome standardisiert und in verschiedene Kategorien unterteilt erfasst werden. Die Anwendung dieses Instruments in prospektiven klinischen Studien soll helfen, den Stellenwert der einzelnen neuropsychiatrischen Symptome bei der Früherkennung einer neurodegenerativen Demenzentwicklung zu erforschen. Die Checkliste wurde bereits in verschieden Sprachen übersetzt und in den entsprechenden Ländern auf seine Validität überprüft.
In dieser Arbeit wurde die Checkliste erstmalig in die deutsche Sprache übersetzt und anhand einer kleinen Kohorte von 21 Personen, welche aufgrund Verhaltensänderungen stationär in der Klinik und Poliklinik für Psychiatrie und Psychotherapie der Universität Regensburg waren, auf seine Anwendbarkeit überprüft.
Die Checkliste konnte bei sämtlichen Teilnehmern und deren Angehörigen vollständig, ohne große Verständnisprobleme, durchgeführt werden. In den Ergebnissen stellte sich besonders der Unterschied der Bewertungen zwischen den Patienten und deren Angehörigen als hervorzuheben dar.
Die MBI-Checkliste kann sich in Zukunft, nicht nur in größer angelegten Studien, als hilfreiches Tool zur Früherkennung und genaueren Differenzierung von neu aufgetretenen Verhaltensauffälligkeiten im Rahmen eines demenziellen Syndroms etablieren.

The onset of dementia syndrome is not only manifested by mild cognitive impairment, but also by behavioral abnormalities and other non-cognitive psychological symptoms, especially in the early stages. This symptom complex has been summarized as Mild Behavioral Impairment (MBI) since 2016.
One of the unresolved difficulties with MBI is the uncertainty as to what extent the presence of such symptoms in an individual patient is indicative of a neurodegenerative disease or rather points to another disease, e.g. a depressive episode, or which of the individual non-cognitive symptoms correlate most with later conversion to manifest dementia.
For this purpose, the MBI checklist was developed to help research groups and clinical professionals diagnose an MBI.
It is a questionnaire with the help of which the symptoms present are standardized and divided into different categories. The use of this instrument in prospective clinical studies should help to investigate the significance of the individual neuropsychiatric symptoms in the early detection of neurodegenerative dementia development. The checklist has already been translated into various languages and tested for applicability in the relevant countries.
In this study, the checklist was translated into German for the first time and tested for its feasibility using a small cohort of 21 people who were inpatients at the Department of Psychiatry and Psychotherapy at the University of Regensburg due to behavioral changes.
The checklist could be completed in all participants and their relatives without any major comprehension problems. In the results, the difference in ratings between the patients and their relatives was particularly noteworthy.
In the future, the MBI checklist can establish itself as a helpful tool for the early detection and more precise differentiation of newly occurring behavioral abnormalities in the context of a dementia syndrome, not only in larger-scale studies.
