Abstract (German)
Das Mammakarzinom ist mit einer Inzidenz von rund 72.000 die häufigste Krebserkrankung bei Frauen in Deutschland. Obwohl die meisten Mammakarzinom-Patientinnen zum Zeitpunkt der Resektion noch frei von Makrometastasen sind, tritt bei ungefähr 20-50 % dieser M0 Patientinnen im weiteren Verlauf ein Rezidiv auf, in dessen Folge der Großteil der Patientinnen verstirbt. Der Erfolg einer adjuvanten Therapie entscheidet sich also daran, inwieweit disseminierte Tumorzellen (DCC) eradiziert werden können oder resistent sind. Aktuell werden Therapieentscheidungen aufgrund von Primärtumor-Analysen getroffen. Außerdem beruht auch die Entwicklung neuer Therapien und die Identifizierung von Zielstrukturen weitgehend auf Analysen der Primärtumoren oder daraus generierter Zelllinien. Dieses Vorgehen steht in einer gewissen Spannung zu dem klaren Befund der prognostischen Relevanz der DCC, die in mehreren Arbeiten nachgewiesen werden konnte. Das Ziel dieser Arbeit war die sichere Identifizierung und molekulare Charakterisierung von DCC, um eine Voraussetzung für die Entwicklung zielgerichteter systemischer, insbesondere adjuvanter, Therapien zu schaffen.
In einem ersten Schritt wurde durch eine Doppelfärbung gegen CK und EpCAM, den am häufigsten zur Detektion von DCC verwendeten Antigenen, eine Koexpression der beiden Proteine in der Mehrheit der Marker-positiven Patientinnen nachgewiesen. Dieser Befund ermöglichte unter der alleinigen Verwendung eines EpCAM-Antikörpers vitale EpCAM+ Zellen aus dem Knochenmark von M0 und M1 Patientinnen zu isolieren, die auch für Genexpressionsstudien verwendet werden konnten. In Kontrollexperimenten zeigte sich, dass EpCAM+ Zellen auch in gesunden Spenderinnen detektiert werden können. Die Anwesenheit von EpCAM+ Zellen im gesunden Knochenmark erschwerte maßgeblich die Detektion von „echten Tumorzellen“. Es wurde zunächst versucht durch histogenetisch präferentiell exprimierte Marker (wie verschiedene Zytokeratin Transkripte und CD45) DCC von autochthonen Knochenmarkzellen zu unterscheiden. Ebenso wurden tumor-assoziierte Transkripte getestet, wobei HER2wt und eine Spleißvariante des HER2 Gens tumorspezifisch waren. Da diese Transkripte lediglich in bis zu 23 % der DCC gefunden wurden, blieb der einfache Klassifizierungsversuch mit Hilfe der Expression ausgewählter Transkripte bei vielen Zellen erfolglos. Deshalb wurde versucht über den Nachweis von chromosomalen Veränderungen mit Hilfe der mCGH die maligne Identität der Zellen zweifelsfrei festzustellen. Bei allen genomisch untersuchten EpCAM+ M1 Zellen und der Mehrheit der EpCAM+ M0 Zellen handelte es sich um disseminierte Tumorzellen mit aberrantem Genom. Dabei unterschied sich die Anzahl der Aberrationen von M0 und M1 Zellen signifikant und es konnten charakteristische chromosomale Alterationen in M1 Zellen detektiert werden. Gegenwärtig bleibt unklar, ob sich auch unter den EpCAM+ Zellen mit balanciertem Profil weitere Tumorzellen befinden, die genetische Veränderungen unterhalb der Auflösungsgrenze der mCGH besitzen. Mit Hilfe einer umfassenden Genexpressionsanalyse konnte weiterhin gezeigt werden, dass EpCAM+ M0 und M1 DCC anhand ihrer Genexpression unterschieden werden konnten. Während Zellen von metastasierten Patientinnen Differenzierungs- und Proliferationsmarker überexprimierten (z.B. RANKL, S100A11), zeigten DCC aus nicht-metastasierten Patientinnen Zeichen einer Adaptation ans Knochenmark. Es konnte eine unerwartet deutliche phänotypische Plastizität der DCC im Knochenmark aufgedeckt werden. Die sichere Identifizierung von aberranten DCC und die Verfügbarkeit ihrer Genexpressionsprofile wird nun erlauben, differenzierende Gensignaturen für die hämatopoetischen EpCAM+ Zellen zu definieren.
Zusammenfassend lässt sich festhalten, dass die in dieser Arbeit begonnenen vergleichenden Analysen von Genom und Transkriptom einzelner DCC die Tür für ähnliche Studien zu metastatischen Vorläuferzellen öffnen. Dies könnte in der Zukunft zu einer Veränderung der Diagnostik der minimalen Resterkrankung, der Suche nach therapeutischen Zielstrukturen und unserem Verständnis der generalisierten Krebserkrankung führen.

With an incidence of about 72,000 breast cancer is the most common cancer among women in Germany. Although most of the breast cancer patients are still free of macro-metastases at the time of resection, disseminated cancer cells (DCC) remain in the body of patients. In about 20-50% of these patients DCC may develop metastases even after several years that will in most cases result in the death of the patient. DCC must therefore be considered as precursors of metastases and thus they represent the target cells of adjuvant therapies. Nevertheless, treatment decisions and the development of new therapies as well as the identification of target structures are currently based upon primary tumor analysis. This approach is not in agreement with the prognostic relevance of DCC, which has been demonstrated by several groups. The aim of this work was to reliably identify and molecularly characterize DCC to create a basis for the development of targeted systemic adjuvant therapies.
In a first step, a double staining with CK and EpCAM, the antigens most commonly used for the detection of DCC, showed co-expression of both proteins in the majority of marker-positive patients. This finding allowed us to isolate viable EpCAM+ cells from the bone marrow of M0 and M1 patients by using an EpCAM antibody and enabled further gene expression studies. Further, we showed that EpCAM+ cells can be detected in healthy donors. The presence of EpCAM+ cells in the bone marrow of cancer-free women hampered the detection of "real tumor cells." We first attempted to distinguish DCC from native bone marrow cells by the preferential expression of histogenetic markers (such as various cytokeratin and CD45 transcripts). Similarly, tumor-associated transcripts, HER2wt and its splice variant, were tested. Since these transcripts have been found in only 23% of the DCC, the classification by the expression of selected transcripts was not successful. Therefore, an attempt was made to reliably determine the malignant identity by the detection of chromosomal alterations using mCGH. All tested EpCAM+ M1 cells and the majority of EpCAM+ M0 cells were disseminated cancer cells with aberrant genomes. Here, the number of aberrations of M0 and M1 cells differed significantly and characteristic chromosomal alterations in M1 cells could be detected. Currently, it remains unclear whether there are additional tumor cells among the EpCAM+ cells with balanced profile, where genetic changes are below the resolution limit of mCGH. Furthermore, a comprehensive gene expression analysis showed that EpCAM+ M0 and M1 DCC could be distinguished by their gene expression. While cells from metastatic patients overexpressed differentiation and proliferation markers (e.g. RANKL, S100A11), DCC of non-metastatic patients showed signs of adaptation to the bone marrow. The data revealed an unexpectedly large phenotypic plasticity of DCC in the bone marrow. Reliable identification of aberrant DCC and the availability of their gene expression profiles will allow to define discriminating gene signatures of hematopoietic EpCAM+ cells.
In summary, it can be stated that the comparative analysis of genome and transcriptome of single DCC opens the door to similar studies on metastatic progenitor cells. In the future this could lead to changes in the diagnosis of minimal residual disease, the search for therapeutic targets and understanding cancer development.
