Non-physiological angiogenesis is a mutual complication of the proliferative forms of diabetic retinopathy (DR), age-related macular degeneration (AMD) and retinopathy of prematurity (ROP), and can lead to severe vision loss or even blindness. There is a great unmet need for more efficient and at the same time less invasive therapies for these diseases (Chapter 1).
The focus of this thesis was on the development of a nanoparticulate system for the targeted therapy of proliferative diseases of the posterior eye segment. This was realized with lipid nanocapsules (LNC) as versatile carriers for lipophilic drugs and a cyclic RGD-peptide as targeting motif to deliver cyclosporin A (CsA) and itraconazole (Itra) directly to their side of action (Chapter 2). CsA and Itra show promising synergistic antiangiogenic properties in vitro, but due to off-target side effects the free drugs are unlikely to make their way into the therapeutic portfolio for proliferative DR, AMD or ROP. The specific transport to proliferating endothelial cells by nanoparticles could help to overcome these obstacles.
LNC are highly appealing as nanocarriers, as they can be prepared in a solvent-free process with various sizes and surface modifications. Size and surface charge are known to essentially determine a nanoparticle´s fate in the body. In this context, it was an important step in the present work to reveal the exact relation between LNC diameter and the ratio of surfactant and lipid phase in the LNC composition. With this knowledge, particles with defined diameters between approximately 20 and 100 nm could be precisely prepared. Additionally, it was shown that the surface charge of LNC can be switched to positive values by the introduction of an aminated poly(ethylene glycol) (PEG)-cetyl/stearyl alcohol. This might enhance the particle uptake into cells but was shown to come at the cost of a higher toxicity and was, therefore, not used in the further development of therapeutic LNC. A complete and stable encapsulation of CsA and Itra in LNC was achieved (Chapter 3).
The endothelial-cell specific targeting with LNC was realized by the conjugation of cyclo(RGDfC) to a PEGylated phospholipid and subsequent post-insertion into the LNC shell. The extent of RGD-grafting under different reaction conditions was evaluated, and it was demonstrated that previous chemical reduction of the peptide with tris(2-carboxyethyl)phosphine is not necessary and can even hamper the reaction. Utilizing the effect of fluorescence resonance energy transfer (FRET), it could be shown that once post-inserted, the phospholipid stays stably anchored in the LNC shell. This is crucial for the peptide to fulfill its targeting task after systemic application. It was demonstrated in vitro that RGD-grafted LNC are massively taken up into endothelial cells, especially in the presence of serum, and that this enhanced cellular interaction does not adversely affect the tolerability of LNC (Chapter 4).
In the next step the effects of RGD-modified and drug-loaded lipid nanocapsules on endothelial cells were investigated in-vitro. RGD-LNC were able to completely inhibit vascular endothelial growth factor (VEGF)-induced proliferation even without encapsulated drugs, confirming that the c(RGDfC)-peptide not only enables the internalization into the target cells, but also exerts a therapeutic effect. Loading of LNC with Itra and CsA could not further enhance this effect on cultured cells. Also, the influence on the expression of hairy/enhancer of split-related protein 1 (HEY-1) and the angiogenesis associated migratory cell protein (AAMP), two markers for the intracellular effects of CsA and Itra, respectively, was unexpectedly low. One reason might be an insufficient drug release from LNC. But it needs to be kept in mind that, although in-vitro models are undoubtedly useful to gather information about the potential of a new therapeutic approach, their outcome highly depends on the specific experimental conditions. Especially in the tube formation model, a poor reproducibility obstructed significant results (Chapter 5).
Additionally, in-vitro models cannot fully represent the complex pathological mechanisms of proliferative eye diseases and the role of nanoparticle biodistribution so far. For this reason, the in-vivo distribution and therapeutic effect of LNC was further investigated in animal experiments. It was demonstrated that RGD-LNC strongly accumulate in the retinal vessels after intravenous administration. A mouse-model of oxygen-induced retinopathy was utilized to mimic the pathological process of retinal neovascularization. In this model, a single injection of RGD-LNC loaded with cyclosporin A and itraconazole significantly reduced the development of tufting and vasodilatation without impeding retinal reperfusion (Chapter 6).

Pathologische Angiogenese ist ein gemeinsames Kennzeichen der proliferativen Formen der diabetischen Retinopathie (DR), der altersbedingten Makuladegeneration (AMD) und der Frühgeborenen-Retinopathie (ROP). Sie kann zu einer Einschränkung des Sehvermögens bis hin zur Erblindung führen. Obwohl die Entwicklung der Anti-VEGF-Therapie einen Meilenstein in der Therapie dieser Erkrankungen darstellte, wären noch wirksamere und gleichzeitig weniger invasive Behandlungsmethoden für diese Krankheiten weiterhin wünschenswert (Kapitel 1).
Der Schwerpunkt dieser Arbeit lag auf der Entwicklung eines nanopartikulären Systems für die gezielte Therapie proliferativer Erkrankungen des hinteren Augensegments. Dazu wurden Lipid-Nanokapseln (LNC) als vielseitige Träger für lipophile Wirkstoffe und ein zyklisches RGD-Peptid als Targeting-Motiv verwendet, um Cyclosporin A (CsA) und Itraconazol (Itra) direkt an ihren Wirkort zu bringen (Kapitel 2). CsA und Itra zeigen in vitro vielversprechende synergistische antiangiogene Eigenschaften. Jedoch haben die freien Wirkstoffe eine Reihe von Nebenwirkungen an Off-Target-Organen, die einer Anwendung bei proliferativer DR, AMD oder ROP entgegenstehen. Der spezifische Transport zu proliferierenden Endothelzellen durch Nanopartikel könnte diese Hindernisse überwinden.
LNC sind attraktive Transportvehikel, da sie mit Hilfe eines lösungsmittelfreien Prozesses in verschiedenen Größen und mit diversen Oberflächenmodifikationen hergestellt werden können. Größe und Oberflächenladung sind bekannte Einflussfaktoren auf die nanopartikuläre Pharmakokinetik. In diesem Zusammenhang war die Beschreibung der Korrelation zwischen dem LNC-Durchmesser und dem Verhältnis von Tensid- und Lipidphase in der LNC-Formulierung ein entscheidender Schritt der vorliegenden Arbeit. Auf diesem Weg konnten Partikel mit vordefinierten Durchmessern zwischen etwa 20 und 100 nm präzise hergestellt werden. Darüber hinaus konnte die Oberflächenladung der LNC durch die Einführung eines aminierten Poly(ethylenglykol)-cetyl/stearylalkohols (PEG) in den positiven Bereich verschoben werden. Dies könnte die Aufnahme der Partikel in die Zellen verbessern. Es zeigte sich jedoch, dass die positive Ladung mit einer erhöhten Toxizität einhergeht. Daher wurde bei der weiteren Entwicklung therapeutischer LNC auf diese Modifikation verzichtet. Eine vollständige und stabile Verkapselung von CsA und Itra in LNC wurde erreicht (Kapitel 3).
Durch die Konjugation von cyclo(RGDfC) an ein PEGyliertes Phospholipid und die anschließende Insertion in die LNC-Hülle wurde eine Endothelzellspezifität der Partikel realisiert. Die Oberflächenmodifikation wurde unter verschiedenen Reaktionsbedingungen durchgeführt und anschließend quantifiziert. Dabei wurde gezeigt, dass eine vorherige chemische Reduktion des Peptids mit Tris(2-carboxyethyl)phosphin nicht zwingend notwendig ist und die Reaktion sogar behindern kann. Mit Hilfe des Fluoreszenz-Resonanz-Energietransfers (FRET) konnte gezeigt werden, dass das Phospholipid nach der Post-Insertion stabil in der LNC-Hülle verankert bleibt. In vitro wurde belegt, dass RGD-markierte LNC verstärkt in Endothelzellen aufgenommen werden, insbesondere in Gegenwart von Serum. Die verstärkte zelluläre Interaktion beeinträchtigte die Verträglichkeit der LNC dabei nicht (Kapitel 4).
Im nächsten Schritt wurde die Wirkung von RGD-modifizierten und arzneistoffbeladenen LNC auf kultivierte Endothelzellen untersucht. RGD-LNC waren selbst ohne Wirkstoffbeladung in der Lage, die durch den vaskulären endothelialen Wachstumsfaktor (VEGF) induzierte Proliferation vollständig zu hemmen. Dies zeigt, dass das c(RGDfC)-Peptid nicht nur eine Internalisierung der LNC in die Zielzellen ermöglicht, sondern selbst einen therapeutischen Effekt ausübt. Die Beladung von LNC mit Itra und CsA konnte diese Wirkung auf kultivierte Zellen nicht weiter verstärken. Auch der Einfluss auf die Expression von Hairy/Enhancer of Split-Related Protein 1 (HEY-1) und das Angiogenese-assoziierten migratorische Zellprotein (AAMP), zwei Marker für die intrazelluläre Wirkung von CsA bzw. Itra, war unerwartet gering. Ein Grund dafür könnte eine unzureichende Wirkstofffreisetzung aus den LNC sein. Jedoch ist das Ergebnis der verwendeten in-vitro-Modelle stark von den spezifischen Versuchsbedingungen abhängig (Kapitel 5).
Zudem können In-vitro-Modelle die komplexen Pathomechanismen proliferativer Augenerkrankungen und die Rolle der Pharmakokinetik von Nanopartikeln bisher nicht vollständig abbilden. Aus diesem Grund wurde die in-vivo-Verteilung und therapeutische Wirkung von LNC in Tierversuchen weiter untersucht. Es wurde gezeigt, dass sich RGD-LNC nach intravenöser Verabreichung stark in den Netzhautgefäßen anreichern. Die therapeutische Wirkung wurde in Mäusen mit einer sauerstoffinduzierten Retinopathie untersucht. In diesem Modell reduzierte eine einmalige Injektion mit wirkstoffbeladenen RGD-LNC signifikant das pathologische Gefäßwachstum sowie die Vasodilatation, ohne die retinale Reperfusion zu beeinträchtigen (Kapitel 6).
