Abstract (English)
This study aimed at the development of a new analytical procedure for the qualitative and quantitative determination of antipsychotics with low therapeutic concentrations and reference ranges such as melperone (MLP), benperidol (BPD), haloperidol (HLP), bromperidol (BRP), flupentixol (FLT) and zuclopenthixol (ZLT) in human serum, both individually as well as simultaneously. This objective was successfully achieved. The procedure uses on-line solid phase sample cleanup coupled to high-performance liquid chromatographic (HPLC) separation with ultra violet (UV) detection. The procedure is inexpensive and affordable for TDM routine applications at small scale TDM laboratories, where these substances could not be tested reliably until now. The development and validation of the method was carried out with the analytical columns Luna phenyl-hexyl 3 µm, 150 x 3.0 mm column and Nucleodur CN-RP 3 µm, 150 x 4.6 mm column for butyrophenones and phenothiazines, respectively. The mobile phase was composed of 30 vol% ACN, 13 vol% MeOH, 0.4 vol% TEMED and 56.6 vol% demineralized water. The wavelength for the detection was set at 245 nm and at 230 nm for butyrophenones and phenothiazines, respectively. The detection limit of each substance (0.7 – 7.3 ng/ml) was satisfactorily obtained. The developed method was successfully applied for TDM routine measurements. It has sufficient sensitivity to detect steady-state concentration in patients. The preparation of the clinic-pharmacological interpretation of the measured concentrations for the optimisation of individual therapy was relied on the information given in TDM request form, namely, the co-medications, development of undesired drug effects, duration of the therapy, dosage regimen, time the sample was taken, and indication for monitoring. This study also shows that the serum concentrations of HLP and FLT for smokers and nonsmokers can be controlled through Therapeutic Drug Monitoring (TDM). The results of the clinic ward questionnaires proved that through the application of the less expensive TDM methods in the clinic ward, attending clinicians were able to effectively monitor the serum drug con-centrations of individual patients, make correct therapeutic decisions in time, and were therefore able to begin the therapy without unwanted delay. The developed and validated isocratic HPLC separation methods with the simple procedures were the appropriate methods for the serum determination of the study substances. This work has shown that proper application of TDM in the praxis can lead to a successful patient-oriented, individualized therapy.

Das Ziel dieser Arbeit ist die Entwicklung eines analytischen Verfahrens für die qualitative und quantitative Bestimmung von Antipsychotika mit niedrigen Konzentrationen und therapeutischen Referenzbereichen, wie Melperon (MLP), Benperidol (BPD), Haloperidol (HLP), Bromperidol (BRP), Flupentixol (FLT) und Zuclopenthixol (ZLT) in Humanserum, sodass die Substanzen sowohl einzeln als auch simultan bestimmt werden können. Dieses Ziel wurde erfolgreich erreicht. Das Verfahren verwendet eine Online-Festphasen-Probenreinigung, die mit einer Hochleistungsflüssigkeitschromatographie (HPLC) und einem Ultraviolett-Detektorsystem (UV) gekoppelt ist. Das Verfahren ist günstig und kann bei kleinen TDM-Laboratorien, wo diese Substanzen bis jetzt nicht zuverlässig geprüft werden können, in der Routine angewendet werden. Die Entwicklung und Validation der Methode wurden mit den analytischen Säulen Luna Phenyl-hexyl 3 µm, 150 x 3.0 Mm für Butyrophenone und Nucleodur CN-RP 3 µm 150 x 4,6 Mm für Phenothiazine, durchgeführt. Die mobile Phase besteht aus 30 vol% ACN, 13 vol% MeOH, 0,4 vol% TEMED und 56,6 vol% H2O. Die Wellenlänge liegt bei 245 nm für Butyrophenone und bei 230 nm für Phenothiazine. Die erhaltene Nachweisgrenze der jeweiligen Substanzen (0,7 – 7,3 ng/ml) ist zufriedenstellend. Die entwickelten Methoden wurden erfolgreich im Praxisalltag für das therapeutische Drug Monitoring eingesetzt. Sie haben genügend Empfindlichkeit um die Steady-State Konzentrationsbestimmungen der Patientenproben durchzuführen. Die Erstellung der Klinik-pharmakologischen Befundung aus den gemessenen Serumkonzentrationen basiert auf den angegebenen Angaben auf dem Konbest-Antragsformular wie Ko-Medikationen, unerwünschte Arzneimittelwirkungen und eingenommene Dosis. Diese Arbeit hat auch gezeigt, dass HLP und FLT Serumkonzentrationen von Rauchern und Nichtrauchern durch das Therapeutische Drug Monitoring (TDM) kontrolliert werden können. Die Ergebnisse der Fragebögen haben bewiesen, dass durch die Anwendung der günstigen TDM-Methoden auf der Station, die Serumkonzentrationen der individuellen Patienten effektiv durch die behandelnden Ärzte kontrolliert werden können. Die richtigen therapeutischen Entscheidungen können rechtzeitig getroffen werden und die Therapie kann ohne unerwünschte Verzögerung beginnen. Die entwickelten und validierten isokratischen HPLC-Methoden sind die passenden Methoden für die Serumkonzentrationsbestimmung der Studiensubstanzen. Die richtige Anwendung von TDM in der Praxis kann zu einer erfolgreichen, patientenorientierten und individualisierten Therapie führen.
